XXI Directing Council of the Pan American Health Organization
CD21.R1 The Directing Council
Resolves:

In establishing the contributions of Member Governments and Participating Governments, their assessments shall be reduced further by the amount standing to their credit in the Tax Equalization Fund, except that credits of those Governments who levy taxes on the emoluments received from the Pan American Health Organization by their nationals and residents shall be reduced by the amounts of such tax reimbursements by the Organization.
3. That, in accordance with the Financial Regulations of the Organization, amounts not exceeding the appropriations noted under Paragraph I shall be available for the payment of obligations incurred during the period 1 January to 31 December 1973 inclusive. Notwithstanding the provision of this paragraph, obligations during the financial year 1973 shall be limited to the effective working budget, i.e., Parts I–VI.
4. That the Director shall be authorized to transfer credits between parts of the effective working budget, provided that such transfers of credits between parts as are made do not exceed 10 per cent of the part from which the credit is transferred. Transfers of credits between parts of the budget in excess of 10 per cent of the part from which the credit is transferred may be made with the concurrence of the Executive Committee. All transfers of budget credits shall be reported to the Directing Council and/or the Conference.

CD21.R2 The Directing Council,
Taking into account that Member Governments appearing in the scale adopted by the General Assembly of the Organization of American States are assessed according to the percentages shown in that scale, in compliance with Article 60 of the Pan American Sanitary Code; and
Considering that other Member Governments and Participating Governments are assessed on the basis of percentages which would be assigned to such countries if they were subject to the OAS scale,
Resolves:
To establish the assessments of the Member Governments and Participating Governments of the Pan American Health Organization for 1973 in accordance with the scale of quotas shown below and in the corresponding amounts.

CD21.R3 The Directing Council,
Having considered Official Document 114, submitted by the Director of the Pan American Sanitary Bureau, which contains the proposed program and budget estimates of the World Health Organization for the Region of the Americas for 1974;
Bearing in mind that the proposed program and budget estimates are submitted to the Directing Council as Regional Committee of the World Health Organization for the Americas, for review and transmittal to the Director-General of that Organization so that he may take them into account when preparing the proposed budget estimates of WHO for 1974; and
Noting the recommendations made by the 68th Meeting of the Executive Committee,
Resolves:
1. To approve the proposed program and budget estimates of the World Health Organization for the Region of the Americas for 1974, appearing in Official Document 114, and to request the Regional Director to transmit them to the Director-General of that Organization so that he may take them into account in preparing the WHO budget estimates for 1974.
2. To request the Director-General that, when preparing those proposed budget estimates, he give favorable consideration to increasing the proportion assigned to the Region of the Americas.

CD21.R4 The Directing Council,
Having examined Official Document 114 submitted by the Director of the Pan American Sanitary Bureau, which contains the provisional draft that is to constitute the basis for the preparation of the proposed program and budget estimates of the Pan American Health Organization for 1974, to be considered by the 70th Meeting of the Executive Committee and by the XXII Meeting of the Directing Council; and
Recognizing that the provisional draft of the proposed program and budget estimates contains much-needed projects prepared in cooperation with the national authorities,
Resolves:
1. To take note of the provisional draft of the proposed program and budget estimates of the Pan American Health Organization for 1974, appearing in Official Document 114.
2. To request the Director to use the provisional draft as a basis for the preparation of the proposed program and budget estimates for 1974, after further consultations with Governments to determine their latest desires and requirements with reference to the health priorities of the countries.
3. To request the Executive Committee to make a detailed examination of the revised program and budget estimates for 1974 to be presented by the Director, after further consultation with Governments, and to submit its recommendations thereon to the XXII Meeting of the Directing Council.

CD21.R5 The Directing Council,
Having studied the constitutional basis as well as policies established by the Directing Council for calculating assessments to the regular budget of PAHO, and noting that a constitutional amendment would be required to change the system;
Recognizing that as long as there are Governments in PAHO which are not members of the OAS, the true percentage of each Government's contribution will be less than that shown in the assessment resolution, but that each Government can easily calculate or obtain from the Director its actual percentage; and
Considering that the present situation does not justify a change in the Constitution of the Pan American Health Organization,
Resolves:
To request the Director to furnish annually to the Executive Committee a document similar to that provided to the 68th Meeting of the Committee (Document CE68/12).

Having considered the recommendation of the Executive Committee and the modifications proposed in the Financial Regulations as shown in Document CD21/14;
Taking into consideration that similar changes in the Financial Regulations of the World Health Organization were approved1 by the Twenty-fifth World Health Assembly; and
Bearing in mind the provisions of Financial Regulations 15.1,
Resolves:
To approve the amendments in the Financial Regulations of the Pan American Health Organization as they appear in the annex to Document CD21/14.

Having considered the Annual Report of the Chairman of the Executive Committee, Dr. Baltazar Caravedo, Representative of Peru (Document CD21/19) on the activities carried out from October 1971 to date, during which period the Committee held its 67th and 68th Meetings; and
Bearing in mind the provisions of Article 9-C of the Constitution of the Pan American Health Organization,
Resolves:
To approve the Annual Report of the Chairman of the Executive Committee, Dr. Baltazar Caravedo, Representative of Peru (Document CD21/19), and to thank the Chairman and the other Members of the Committee for the work done during the period covered by the Report.

Resolves:
To approve the Annual Report of the Director of the Pan American Sanitary Bureau for 1971 (Official Document 116) and to commend him and the rest of the staff of the Bureau for the work done during the period covered by the Report.

Having considered the Financial Report of the Director and the Report of the External Auditor for fiscal year 1971 (Official Document 113), as well as Resolution I approved by the Executive Committee at its 68th Meeting,
Resolves:
1. To approve the Financial Report of the Director and the Report of the External Auditor for fiscal year 1971 (Official Document 113).
2. To commend the Director for having consistently pursued long-term financial policies that have enabled the Organization to achieve and maintain a sound financial position.

CD21.R10 The Directing Council,
Having considered the report presented by the Director on engineering and maintenance of hospitals and other health services (Document CD21/10),1 as well as Resolution XIV of the 68th Meeting of the Executive Committee;
Recognizing the importance of hospital maintenance services to ensure good quality of health care delivery; and
Bearing in mind that, to keep pace with the increasing demands for an effective health care delivery system measures must be taken to upgrade maintenance and engineering in all areas of health,
Resolves:
1. To commend the Director for the effective support extended to the Hospital Maintenance and Engineering Center in Caracas, Venezuela, the activities of which have been essential for promoting interest in this field throughout the Region.
2. To urge ministries of health of all the countries of the Region to establish a maintenance unit at the national level and to request financial assistance from the United Nations Development Program (UNDP), whenever necessary; to assign qualified personnel and an adequate budget to each health maintenance activity; and to train personnel to effectively handle and operate the maintenance program within the health service.
3. To recommend to the Director that he continue to promote a regional maintenance and engineering program for medical care services and other health services and that he explore possible national or international sources of financing to help interested countries in the expansion of these services.

Having considered the report of the Committee on the PAHO Award for Administration (Document CD21/17 and Annex);1 and
Bearing in mind the criteria for the PAHO Award for Administration,
Resolves:
To declare Dr. Eduardo Zapata Salazar of Peru the winner of the 1972 PAHO Award for Administration.

CD21.R12 The Directing Council,
Having taken note of Document CD21/16 and annex1 on country reports on health conditions, prepared by the Director of the Pan American Sanitary Bureau; and
Bearing in mind the interest expressed by the countries in continuing to present to Pan American Sanitary Conferences a report on the progress achieved in the health field,
Resolves:
1. To request the countries to send to the Pan American Sanitary Bureau not less than 60 days before the date of the meeting of the Pan American Sanitary Conference a report setting forth the most important developments in the health field in the past four years, with special emphasis on changes and innovations in the organization and administration of health services and on novel aspects of programs in operation which are of general interest.
2. To recommend to the XIX Pan American Sanitary Conference that during plenary sessions, the delegates limit their speeches to a statement of the most important points in the above-mentioned report or of any other information they consider to be of interest to the Conference. The statement shall not exceed four double-spaced typewritten pages and the time for reading it shall not exceed 10 minutes. For the purposes of this presentation, delegates are to be entered previously on the list of speakers.
3. To request the Director of the Pan American Sanitary Bureau to communicate with the health authorities of the countries not less than six months before the date of the meeting of the Conference reminding them of the terms of this resolution.

CD21.R13 The Directing Council
Having heard the statement by the Director of PASB on the III Meeting of Ministers of Health of the Americas, held in Santiago, Chile. 2 - 9 October 1972; and
Considering that the recommendations1 made by the Meeting constitute valuable guidelines on the direction to be taken by the programs of the countries during the next decade.
Resolves:
1. To incorporate into the policy of the Pan American Health Organization the recommendations of the III Meeting of Ministers of Health of the Americas.
2. To request the Director of PASB to study the implications of those recommendations and the consequent modifications in the objectives and priorities of the program of the Organization, and to report thereon to the XXII Meeting of the Directing Council.
3. To recommend to the countries that, within their planning process, they identify the priority health problems and establish objectives for each of them in accordance with the manpower, physical, and financial resources available, taking into account the regional health goals.
4. To request the Director of PASB to convene as soon as possible a working party of personnel responsible for planning and information and of health economists, with a view to designing an evaluation system that can be adapted to the conditions of each country and still be flexible enough to give comparable results and in turn permit a continent-wide evaluation of the achievements of the decade.
5. To recommend to PAHO that, in consultation with experts of the countries, general guidelines be drawn up for determining the present financing of health investments and the changes required to carry out the plans and programs envisaged in the Ten-Year Health Plan for the Americas, 1971–1980.
6. To suggest to the health authorities of the countries that they initiate cost studies of their health services and, and whenever possible, cost-benefit studies, particularly in areas with the largest investment.
7. To recommend to PAHO that it prepare the necessary designs to ensure comparability of the cost studies.
8. To recommend that PAHO furnish advisory services to countries requesting them, in all matters relating to the financing of the health sector, such as the financial analysis of the sector, analysis of health expenditures, programming of investments, management and financing of specific projects and of external credits, so that the countries will gradually come to know what financial resources are earmarked for health and how they are related to the benefits obtained.

CD21.R14 The Directing Council
Elected Haiti, Mexico, and Panama to the Executive Committee upon termination of the periods of office of Guyana, Peru, and Venezuela, and thanked the Governments of these countries for the services rendered to the Organization by their representatives on the Committee.

CD21.R15 The Directing Council,
Considering that at present approximately 99 per cent of the funds for the regular budget of the Pan American Health Organization are derived from the quota contributions of Member Governments and Participating Governments, i.e., that income from other financial sources represents only just over 1 per cent of the total regular budget;
Bearing in mind that, among other factors, the high rate of natural population increase in the Hemisphere, the commitment entered into by the Governments at the III Special Meeting of Ministers of Health of the Americas to attain realistic but ambitious goals, and the continuous increase in the production costs of health services due to scientific and technological development, are sufficient reasons for considering the problem of the necessary increase in the PAHO budget a matter of the utmost seriousness and urgency;
Recognizing that it is not possible for the burden of that increase to continue to fall almost exclusively upon the quota contributions of the Member Governments and Participating Governments, from which it can be deduced that the most advisable and feasible course is to look for means of achieving a rapid and substantial expansion in the sources of financing in addition to quota contributions, so that the major part of the budgetary increase would be derived from those new sources, apart from the historical process of growth in quota contributions that has been taking place and will continue to occur; and
Considering that in this regard the efforts which the Pan American Sanitary Bureau has been making toward establishing a financial institution to promote the raising of supplementary financial resources for the Organization's budget are commendable,
Resolves:
1. To instruct the Director of the Pan American Sanitary Bureau to establish a special group of experts to study the problem of obtaining financial resources to supplement the quota contributions of Member Governments and Participating Governments, and that the group propose concrete alternative solutions, feasible and capable of immediate application, to the 70th Meeting of the Executive Committee.
2. To request the 70th Meeting of the Executive Committee consider the report of the special group of experts and on the basis thereof present suitable recommendations to the Directing Council at its XXII Meeting so that the most appropriate alternative solution may be implemented as rapidly and effectively as possible.

CD21.R16 The Directing Council,
Having considered the proposed program and budget estimates of the World Health Organization for the Region of the Americas for 1973 and 1974, contained in Official Document 114, as well as the proposals contained in Document CD21/4; and
Recognizing the advisability of projecting probable program needs into future years,
Resolves:
To recommend to the Director-General that he include in the program and budget estimates of the World Health Organization for 1975 the proposed program for the Region of the Americas as detailed in Document CD21/4.

CD21.R17 The Directing Council
Having considered the Final Report of the V Inter-American Meeting, at the Ministerial Level, on Foot-and-Mouth Disease and Zoonoses Control (Document CD21/5);
Recognizing the importance of strengthening the coordination of activities carried out by the ministries of agriculture and of health in the field of human and animal health;
Mindful of the need for continued efforts in manpower development in animal health planning;
Being aware of the seriousness of the public health hazards resulting from the indiscriminate use of pesticides, particularly in view of the lack of adequate legislation or of analytical laboratories to aid in the needed remedial action; and
Noting that the forthcoming completion of the Pan American Highway through the Darien Gap will increase the risk of introduction of certain diseases of human and animal significance into areas now free of them,
Resolves:
1. To take note of the Final Report of the V Inter-American Meeting, at the Ministerial Level, on Foot-and-Mouth Disease and Zoonoses Control (Document CD21/5).
2. To emphasize the advisability of having representatives of the ministries of health attend future Inter-American Meetings on Foot-and-Mouth Disease and Zoonoses Control, in order to obtain a better coordination of activities in this field.
3. To express its thanks to the ministries of agriculture for their endeavors in the control of these diseases of human and animal health significance, which, if allowed to persist, would hinder the economic and social development of the countries.
4. To recommend to the Governments that they give serious consideration to the impending hazards to human and animal health resulting from the application and indiscriminate use of pesticides, and that they put into practice the recommendations contained in Resolution XV approved at the V Inter-American Meeting on Foot-and-Mouth Disease and Zoonoses Control.
5. To reaffirm its support of the Pan American Foot-and-Mouth Disease Center and the Pan American Zoonoses Center and the programs they are conducting.
6. To emphasize the need for greater cooperation between the ministries of agriculture and of health and international organizations in order to more effectively control animal diseases as a feasible measure for improving human health in the Hemisphere.

CD21.R18 The Directing Council,
Having considered the report on the collection of quota contributions (Document CD21/12 and Add. I); and
Having received and considered the report of the working party on the application of Article 6.B of the Constitution of the Pan American Health Organization, which indicated:
a) That two countries, Bolivia and Haiti, have made payments in 1972 toward fulfillment of plans for payment of arrears within a definite period, previously approved by the Directing Council; and that both intend to make further payments before the end of this year;
b) That two countries, Paraguay and Uruguay, have made partial quota payments in 1972, and each intends to make further payments so as to complete their quotas for 1972. In addition, each Government proposes to adopt a plan, starting in 1973, to pay an amount equal to its current quota plus 10 per cent of the arrears outstanding at the end of 1972;
c) That the Government of the Dominican Republic plans to pay the amount of its current quota, US$54,065, before the end of 1972. Furthermore, it proposes to adopt a plan, starting in 1973, to pay an amount equal to its current quota plus 10 per cent of the arrears outstanding at the end of 1972,
Resolves:
1. To take note of the report on the collection of quota contributions (Document CD21/12 and Add. I).
2. To thank the Governments that have already made payments in 1972, and to urge all Governments to pay remaining balances of arrears and current year quotas before the end of the year.
3. To note with appreciation the payments made by Bolivia and Haiti, as well as their intention to make further payments in fulfillment of their respective plans for payment of arrears within a definite period.
4. To note with appreciation the intention of Paraguay and Uruguay to make further payments in 1972 which, together with amounts already paid, will equal the current quota.
5. To note the intention of the Dominican Republic to pay its current quota before the end of the year.
6. To approve the respective plans proposed by the Dominican Republic, Paraguay, and Uruguay for payment in future years of their current quotas plus 10 per cent of the arrears outstanding at the end of 1972.
7. To permit the above-mentioned five Governments affected by Article 6.B of the Constitution to vote in the XXI Meeting of the Directing Council.
8. To request the Director of PASB to continue to inform the Governments of any balances due and to report to the XXII Meeting of the Directing Council on the status of the collection of quotas.

Having considered Document CD21/3, in which the Director of the Pan American Sanitary Bureau submitted to the Regional Committee of the World Health Organization for the Americas some of the resolutions approved by the Twenty-fifth World Health Assembly and by the Forty-ninth and Fiftieth Sessions of the Executive Board:1 and
Bearing in mind Annex VIII of Document CD21/3 relating to the Twenty-fifth Anniversary of the World Health Organization,
Resolves:
1. To take note of the following resolutions of interest to the Regional Committee:
WHA25.26: Organizational Study by the Executive Board on Medical Literature Services to Members.
WHA25.35: Community Water Supply.
WHA25.39: New Zone Office Building in Brasília.
WHA25.42: Training of National Health Personnel.

CD21.R20 The Directing Council,
Having considered the report on the activities of the Pan American Center for Health Planning (Document CD21/20), presented by its Director in compliance with Resolution XXIII of the 66th Meeting of the Executive Committee;
Having considered the report of the First Meeting of the Technical Advisory Committee of the Center, held in 1971;
Bearing in mind the programming of the second phase of the Center, in which its program of work is expanded and reoriented with a view to completing the transfer of training and research responsibilities to the countries, and to strengthening and expanding its information services; and
Recognizing that the Center is complying with the general guidelines for the establishment, operation, and financing of multinational centers, established in Resolution XXXIII of the XVIII Pan American Sanitary Conference,
Resolves:
1. To commend and thank the Director of the Center for the training, research, and information activities the Pan American Center for Health Planning is providing in support of the countries.
2. To thank the United Nations Development Program (UNDP) for the financial assistance it is giving to the Center through Program RLA/68/083.
3. To endorse the new orientation of the activities of the Center for a second phase, especially the emphasis on information services and the way in which they will be used to continue to support and orient the training and updating of specialized manpower and the research they are carrying out in the countries on planning problems.
4. To recognize the importance of maintaining the level of efficiency of the Center, especially for carrying out its second phase, and to support the Director of the Pan American Sanitary Bureau in the negotiations that are being conducted with the UNDP for the financing of the second phase of Program RLA/68/083.
5. To request the Director of PASB to report to the Directing Council on the projected activities and the financial needs of the Center as well as on the negotiations being conducted with the United Nations Development Program.

To designate the representatives of Brazil and of Trinidad and Tobago to speak on behalf of the Region of the Americas at the commemorative session of the Twenty-fifth Anniversary of WHO to take place at the Twenty-sixth World Health Assembly.

Having heard with gratification that the United Nations Development Program has approved the new regional project of the Pan American Zoonoses Center; and
Recognizing that in accordance with the plan of operations of this regional program it is necessary to establish a coordinating committee composed of three representatives of the ministries of health,

To elect the Governments of Uruguay, the United States of America, and Venezuela to serve on the coordinating committee for the regional program of the Pan American Zoonoses Center,

Taking into account the report of the Director of the Pan American Sanitary Bureau on the proposed agreement between PAHO/WHO and the Organization of Central American States (ODECA) (Document CD21/15); and
Bearing in mind the authorization granted by the Executive Committee to the Director at its 68th Meeting, to continue negotiations with ODECA in order to formalize relations between the two organizations,
Resolves:
To authorize the Director of the Bureau to continue the above-mentioned negotiations and to sign eventually the agreement between PAHO/WHO and the Organization of Central American States.

Having considered the report of the cost-benefit study on the prevention of Aedes aegypti-borne diseases in the Americas (Document CD21/9),1 prepared by a specialized firm in accordance with Resolution XIII of the XX Meeting of the Directing Council,
Resolves:
1. To note the conclusions and recommendations of the study.
2. To request the Director of PASB to make known the comments and observations made on the study by experts from other agencies.
3. To urge the Governments of the countries still infested to decide without delay to eradicate Aedes aegypti from their territories, in accordance with the resolution2 approved at the I Meeting of the Directing Council (Buenos Aires, 1947).
4. To note with satisfaction the statement by the representatives of the countries which have declared their intention to take the necessary measures for preventing reinfestation of neighboring countries.
5. To express its thanks to the Governments of the United States of America and of Trinidad and Tobago for their generous voluntary contributions toward the financing of the study.
6. To request the Director to convene a group of experts in various disciplines connected with this vector and the diseases it transmits, in order that the group may issue recommendations in the light of new knowledge and experience.

CD21.R27 The Directing Council
Taking into account the importance of health problems in frontier regions common to two or more countries; and
Considering that for the control of the diseases which affect border areas it is essential to carry out synchronized, coordinated measures financed simultaneously by the countries involved,
Resolves:
1. To request the assistance of the Pan American Sanitary Bureau in promoting with the Governments having common health problems along their borders that they study them jointly and develop coordinated activities, and that they make available the necessary financial assistance for the control of diseases affecting border areas or for programs financed jointly by the interested parties.
2. To recommend that, when necessary, the health authorities of the countries establish groups with the specific function of giving priority attention to border problems.
3. To request those countries which have joint border programs to meet periodically in order to evaluate the activities developed.
